Submitting an Application
1. Introduction
In order to make a submission for funding, you must agree to abide by all of these Additional Terms and Conditions as well as the terms and conditions applicable to all users of the Web Site set forth in the Terms and Conditions of Use. Please read these Additional Terms and Conditions carefully before submitting any information. By making a submission, you indicate your acceptance of these Additional Terms and Conditions and the terms and conditions applicable to all users of the Web Site set forth in the Terms and Conditions of Use. ISF may revise these Additional Terms and Conditions at any time by posting an update to this page. You should visit this page periodically, because your continued use of the Web Site following the posting of changes to these Additional Terms and Conditions will mean you accept those changes. The terms "you" and "user" as used herein refer to all individuals and/or entities.

You understand that the information that you are submitting is for informational purposes only and the information is neither an offer to sell nor a solicitation of an offer to buy any securities. No rights or expectations of funding from the Indiana Seed Fund are created. You are also aware of the possible applicability of Section 5(a) and (c) of the Securities Act of 1933 to a public offering of securities which does not come within the terms or guidelines set forth in Rule 502(a) of Regulation D.
3. Non-Confidentiality
You agree not to submit any confidential information to the Web Site. You also agree that other users of the Web Site may use any information that you submit to the Web Site for any purpose whatsoever and may disclose or otherwise make available any of such information to anyone, including third parties.
Investment Criteria:
All proposals will be evaluated with the following criteria in mind:
- Proposer must be a life sciences company.
- Proposer shall identify a credible target market.
- Sales potential for the company's products must be significant and demonstrable.
- The product or service must be novel and appropriately protected or protectable.
- The company's leadership must be capable of guiding the company to its next set of reasonable and identified milestones.
- Proposer must clearly describe its plan for utilizing the funds.
- Proposer must have or have demonstrated potential to attract additional third-party funding.
- Operations need not be fully established; however, the company must be organized and located in Indiana.
SUBMISSION GUIDELINES
1. Company Information
- Name
- Name of company contact
- Address, email, telephone number
- Web page (if applicable)
- Amount requested
2. Market Information
- Description of market need to be met, together with any supporting data
- Description of customer to be served
- Customer validation to date for proposed product
- Noteworthy trends in target market
- Size and growth rate of target market
- Major competitors in target market (indicate those which have received venture financing)
3. Product Information
- Description of product
- Intellectual property/patent portfolio
- Cost/benefit analysis compared to competitors' validation of the technology
- Status of product development and projected timeline to market
- Description of IP and related ownership
4. Management
- Relevant experience and detailed biographies of key participants in management
- Relevant experience and detailed biographies of members of the Board of Directors and Advisory Board(s) (if any)
- Key vacancies and plans to fill them, including timing of additions
5. Financial Information
- Copy of most recent financial statements
- Investment to date by principals
- Attach completed Word document to email and submit to BioCrossroads
- Indicate how much financing will be required to fund that forecast, how much being sought in this round and any fundraising activities completed to date
Length of submissions: maximum 5 pages.
Requirements for submitting an application:

- Fill in the required information in the Word document
- Attach completed Word document to email and submit to indianaseedfund@biocrossroads.com
- You may also attach your business plan as a separate document *
- Please note, submission of a business plan only will result in non-application. The 'Indiana Seed Fund Submission Guidelines' Word document must be filled out and sent for successful application.
